OBJECTIVE To evaluate the effects of Dydrogesterone (Duphastone ) on serum changes of hCG and progesterone and the prognosis of the pregnancies in patients with recurrent and threatened abortion. Material and METHOD: From June, 1996 to May, 1997, one hundred and thirty patients of threatened abortion who had the history of repeated spontaneous abortion (>2 times) were chosen for study. Total patients were divided into the study group (N=77) who had received Dydrogesterone and the control group (N=53). Dydrogesterone were administered orally, 20 mg/day. The serum hCG and progesterone were serially checked over 48 hours interval, every weeks during her stay in the hospital. The outcomes of pregnancy were also followed. RESULTS: There were no significant differences in age, number of gravity and prior spontaneous abortion between study group and control group. In control group, the 48 hours-increament (%) of the serum hCG at 6th, 7th, 8th, 9th, 10th, and 12th gestational weeks were -2%, 29%, 10%, 27%, 16%, and 20% respectively. The 48 hours-increament (%) of the serum progesterone in the control group at 6th, 7th, 8th, 9th, 10th, and 12th gestational weeks were -33%, 2.1%, -14%, 7.4%, -12%, and -20% respectively. In study group, the 48 hours-increament of hCG at 5th, 6th, 7th, 8th, 9th, 10th, and 11th gestational weeks were 38%, 18%, 21%, 1.7%, -7%, 0.7%, and -10% respectively. The 48 hours-increament of serum progesterone at 5th, 6th, 7th, 8th, 9th, 10th, and 11th gestational weeks were 18%, -7%, -4%, 0.4%, 0.2%, 6% and 10% respectively. The preterm birth and abortion rate were somewhat lower in study group, however, there were no statistical significance. CONCLUSION: There were stable increament (around 20%) of serum hCG after admistration of oral dydrogesterone in the study group until 7th gestational weeks. These data suggest that oral dydrogesterone would be useful for increament of serum hCG especially early gestational age in patients with threatened and recurrent abortion. |
