It has been known that most ovarian hyperstimulation syndrome (OHSS) is associated with the use of exogenous gonadotropins to induce multiple ovulation. However, OHSS-like condition could be developed rarely in spontaneous ovulatory cycle. We report a case of OHSS in a spontaneously pregnant woman. A 35-year-old woman conceived in a natural ovulatory cycle. She visited our hospital initially at 6th weeks of gestation for antenatal check without specific problem. At 12th weeks of gestation, she complained of low abdominal pain, abdominal distension, and dyspnea. Ultrasonography revealed bilateral enlarged ovarian cysts with small amount of ascites. The serum concentration of estradiol (E2), progesterone, and renin were >5,000 pg/ml, >50 ng/ml, and 8.1 ng/ml/hr, respectively on the day of admission. Conservative management was adopted to the patient with daily intravenous 5% Dextrose/Sodium 1,000 ml, 20% albumin 100 ml, and Dextran (Rheomacrodex-D) 500 ml for about 10 days until she dischaged. The pregnancy progressed to term and she delivered of a normal female infant at 42th week of gestation. As OHSS is self-limited disease, the conservative management may be adequate for initial approach. |
